I'm a bit baffled by the colored spots. Do the blurry grey ones not go away if you increase minimum ray samples?

With the old H10 PBR I occasionally got white hard holes if I used depth of field, but not colored ones as far as I can recall.

To rule out mplay video card issues, I'd try rendering to disk directly to see if it makes any difference.

i ve tried to render to disk directly and the same artefacts appear .... quadroFX 4800 here ?! similar artefacts appear in other extra planes like for example reflection added in H11 for PBR ... hope this will be fixed soon for a nice PBR workflow !

by the way the gray smooth dots are correct geoshadows (although with some grain if you were pointing on those macha?)

did you come to a solution with the white holes ?
